**This form is mandatory for each family to complete before arriving at the first practice session.**

I will complete an athlete check-in form provided by the Mustangs. This is a mandatory list of questions.

I have provided my email, home address and phone number to the club registrar. This is to facilitate contact tracing in the event of an illness with one of the athletes or coaches.

I will maintain 3 meters of distance between other athletes at all times.

I will maintain hand hygiene using isogel if in contact with any equipment or surfaces during the practice. The coaches will have isogel at practice but it is recommended each athlete bring their own.

If I have any respiratory symptoms, fever >38 degrees Celsius, or other Covid symptoms l will not come to practice but notify the head coach immediately.

I agree to abide by the protocols above, and by not doing so will be subject to immediate suspension from practice with a follow up review to determine future involvement with club activities.
